Your spine can shorten and curve forward due to the collapse or compression of the vertebral bone resulting in pain and a kyphotic (hunched-over) deformity.

Lumbar kyphoplasty is a procedure used to improve lumbar vertebral compression fractures by stabilizing the fractured vertebra, restoring vertebrae height, and relieving the associated pain.

How Lumbar Kyphoplasty Works

Lumbar kyphoplasty works by stabilizing the fractured vertebra, restoring vertebrae height, and preventing them from collapsing again. It also helps to reduce associated pain and increase mobility in your spine.

Preparations for a Lumbar Kyphoplasty

  • Before having lumbar kyphoplasty, you will have to be evaluated by your doctor. Your medical history will be reviewed. Some tests will be ordered.
  • It is typically recommended to quit smoking and drinking alcohol.
  • About a week before the procedure, you may need to temporarily stop some medications, such as nonsteroidal anti-inflammatory drugs (NSAIDs) and blood thinners. The reason for stopping these medications is that they could increase the risk of bleeding and other complications.
  • For about 6-7 hours before the procedure, you are not allowed to eat or drink anything.
  • If you’re pregnant, have any medical conditions or allergies, or are currently on any medications, inform your doctor.

Lumbar Kyphoplasty Procedure 

During a lumbar kyphoplasty procedure, you will be asked to lie down on your belly on a procedure bed so your surgeon can access your back. You may be given a local or general anesthetic. A general anesthetic puts you asleep throughout the procedure, so you don’t feel pain. A local anesthetic help to numb pain so you don’t feel pain throughout the process, but you will be awake.

After the anesthetic is administered, you may need to wait some minutes for the drug to start working. A small incision will be made at your back’s lumbar (lower) part. A needle will be inserted through the incision into the lumbar vertebral compression fracture site using X-ray guidance.

When the needle is in the correct position, a small device called a balloon to tamp will be passed through the needle into the fractured vertebra. The balloon tamps will then be inflated to restore vertebra height and create a cavity in the broken vertebra. After restoring vertebra height and creating a hole, the balloon tamps will be removed, leaving a crater.

Orthopedic cement will then be injected into the cavity. The cement quickly hardens to strengthen and solidify the damaged vertebra and prevent the vertebra from collapsing. Restoring vertebra height and stabilizing the fractured vertebra provides pain relief.

After the Procedure 

Immediately after the procedure, you will be taken to a recovery room and closely observed for an hour or two. If there are no complications, you will be allowed to go back home.

You are not allowed to drive after the surgery. You will need to arrange for someone that will guide you back home.

Some patients experience pain relief immediately after the procedure, while some experience pain relief within two days.

You can return to your normal daily activities. Avoid strenuous activities for at least six weeks after your surgery.

You may resume driving with your doctor’s permission a few days or weeks after your surgery, depending on your health status and the medications you’re taking.

You will need to visit your doctor’s office for a follow-up visit, typically a few weeks after the procedure. Your doctor will check how well you are recovering.

Who is a good candidate for lumbar kyphoplasty?

A good candidate for lumbar kyphoplasty is a person;

  • Suffering from chronic pain that is related to lumbar vertebral compression fractures. A person suffers from painful lumbar vertebral fractures caused by osteoporosis, bone marrow cancer, metastatic cancer, or benign vascular tumors.
  • With a reduced or limited ability to move and function due to lumbar vertebral fractures and associated pain.

Lumbar kyphoplasty provides better results when it is performed less than eight weeks after a lumbar vertebral compression fracture is diagnosed. Significant healing hasn’t occurred in less than eight weeks. Hence, having the surgery during this time will allow for better vertebral height restoration.

Lumbar kyphoplasty should not be performed after 12 or more weeks after a vertebral fracture. This is because significant healing has already occurred during this time, and results from lumbar kyphoplasty may not be good.

Individuals with specific conditions such as bone infections, tumors in their spinal canal, bleeding disorders, and allergies to the medications used during the procedure are not good candidates for lumbar kyphoplasty.

If you’re pregnant, you’re not allowed to have lumbar kyphoplasty.

Potential Complications of Lumbar Kyphoplasty

Lumbar kyphoplasty is a minimally invasive procedure. It has lower risks than major or invasive surgeries. Complications are common, but as with most surgical procedures, some severe complications and adverse effects may occur.

Infection, excessive bleeding, and adverse reactions to anesthesia used during the surgery are common side effects that occur following lumbar kyphoplasty.

1. Bone cement leakage

This is because the bone cement is injected with less pressure, and it’s more likely to stay within the space of the cavity created by the inflated balloon.

2. Allergic reaction to bone cement or other agents used during the procedure

Some people may have an allergic reaction to the bone cement or the solution used to see the balloon more clearly via X-ray. Allergic reactions don’t occur in everyone.

3. Pulmonary embolism

If bone cement leaks out and gets into a vertebral vein, the bone cement that leaks out can potentially travel to the lung and obstruct an artery, causing pulmonary embolism. Pulmonary embolism can be life-threatening; hence, it requires immediate treatment.

4. Pain that persists or worsens

In some cases, lumbar kyphoplasty can fail to relieve the pain or may even make the pain worse. If the bone cement leaks out onto a nerve root or the spinal cord, it could cause worsened pain.

5. Nerve damage

Nerve damage can occur when the bone cement leaks into a nerve root. This may lead to numbness, weakness, or tingling.

6. Paralysis

Paralysis may occur in sporadic cases. Paralysis can occur if the spinal cord or a nerve root becomes damaged. If the bone cement leaks out excessively onto the spinal cord or a nerve root, it could lead to paralysis.

In rare cases, heart attacks, cardiac arrest, or stroke can also occur.
